Need to View or Print Data Entry Forms
This guide will help you view or print the data entry forms previously provided along with your shipped samples.
There are two main ways to access the data entry forms based on your intended use. Via claiming the test for entry or through the Data Entry Library.
Note: if you claim a test for entry this will lock it to your account and will have to be unassigned/reset to pass to another user. Industry Clients are encouraged to use the How to Share a Claimed Cycle with a Group walkthrough, or contact CTS for assistance in resetting the cycle. Claim the Test for Entry
If you are the user who will be entering and submitting data, the option to print a copy of the Data Entry Forms are found within your My Data Entry page after you have claimed the test. (How to to add a Cycle to your account and Claim for a Group (Industry) / How to Claim a Test (Forensics))
Note: To claim a test you will need the Participant Code / LabCode and WebCode. These are provided to the Primary Contact as an excel attachment with the Shipping Notification. They are also preloaded for the Lab Account Owner (Primary Contact) or users who have been granted permission to access them. (How to Set Data Entry Module Permissions)
Each Cycle or Test ID will have a block of icons which includes the Email PDF and Printer Friendly functions.
Industry this is seen per LabCode for either the Cycle or the Test Series:

Use the Data Entry Library
If you are not the user who will be entering and submitting data, the Data Entry Forms can be accessed through the Data Entry Library. However, you must be the Lab Account Owner (Primary Contact) or have been granted permission to access the Data Entry Library. (How to Set Data Entry Module Permissions)
Click into the Data Entry Library, select the year, select the Program/Discipline, and then select the Cycle or Test ID and you will get to a listing of the Participant Codes assigned to your laboratory.
Industry – You can request a PDF version of the entire cycle to be emailed to you by checking the box and clicking the Email PDFs button.
Forensics – You can access a printer friendly version by clicking on the Participant Code, or you can request a PDF version to be emailed to you by checking the box and clicking the Email PDFs button.
